When Is a Filling Not Enough?
Finding out you have a cavity or damaged tooth does not automatically mean the solution will be a filling. Fillings are an excellent way to repair many teeth, especially when the damaged area is relatively small and plenty of healthy tooth structure remains. However, as damage becomes larger, the tooth may need more support than another filling can provide.
At Preferred Dental Care, we plan restorative treatment around the individual patient. The goal is to repair damage while preserving as much healthy natural tooth structure as possible and choosing a restoration that can support the tooth for the long term.
The Size of the Damage Matters
A filling works by replacing tooth structure that has been lost to decay or damage. When the affected area is small to moderate, the remaining natural tooth can usually provide enough support for the filling. As more of the tooth becomes compromised, that balance begins to change.
A very large filling may leave only thin sections of natural tooth around it. Those weakened areas still have to withstand everyday biting and chewing forces. At some point, simply replacing an old filling with an even larger one may not provide the strength the tooth needs.
Location matters too. A restoration on a back tooth must withstand different chewing forces than one located elsewhere in the mouth. The location and extent of the damage both help determine the appropriate treatment.
When an Inlay or Onlay May Be an Option
Sometimes a tooth falls somewhere between needing a filling and needing a full crown. Preferred Dental Care offers inlays and onlays for situations where a cavity or damaged area is too large for a simple filling but does not necessarily require a crown. These restorations are custom-made to fit the prepared portion of the tooth.
An inlay fits within the chewing surface of the tooth. An onlay can cover a larger portion of the tooth, including one or more of the raised cusps. Because they restore only the areas that need additional support, inlays and onlays can provide a useful middle ground when preserving healthy natural tooth structure is still possible.
When a Crown Provides More Support
A crown may be recommended when a tooth has lost enough structure that a filling, inlay, or onlay would not provide sufficient protection. Instead of repairing only one section, a crown covers the visible portion of the tooth. This helps reinforce weakened tooth structure while restoring its shape and function.
Preferred Dental Care also uses CEREC technology to create full crowns, inlays, and onlays. The practice states that these restorations are made from compressed porcelain and can be created in the office using digital technology.
The purpose is not to use the largest restoration possible. It is to choose the level of support that matches what the tooth actually needs.
Preserving What Is Healthy
Whenever possible, keeping healthy natural tooth structure is an important part of restorative dentistry. That means treatment is not simply about removing damaged areas and replacing them with dental materials. It also involves evaluating how much healthy tooth remains and determining how that structure can best be protected. Sometimes a filling accomplishes exactly that. In another situation, an inlay or onlay may provide greater support without requiring a full crown. For a significantly weakened tooth, a crown may be the more appropriate way to protect what remains.
Why Similar Teeth May Need Different Treatment
Two patients may both hear that they have a damaged molar yet leave with completely different treatment recommendations. Even two teeth in the same mouth can have different needs. One may have a small area of decay surrounded by strong enamel, making a filling appropriate. Another may have an old, large restoration with very little healthy tooth structure left around it. A third may have damage that extends across one or more cusps. The diagnosis may sound similar, but the condition of each tooth is not. That is why restorative dentistry should never feel one-size-fits-all.
